Overview
Course Description

This course introduces a human factors approach to maritime incident investigation. Through case studies and exercises, participants learn to gather facts objectively, identify human and organizational contributors, conduct effective interviews, and determine root causes to prevent recurrence. The emphasis is on learning and improvement rather than blame.  

Outcomes

By the end of the course, participants will be able to: 

  1. Explain the purpose of incident investigation.  
  2. Recognize common human factors and cognitive biases.  
  3. Distinguish immediate causes from root causes.  
  4. Apply the Five Whys technique.  
  5. Conduct interviews using appropriate questioning techniques.  
  6. Demonstrate an objective, fact-finding mindset.  
  7. Foster psychological safety during investigations.
